Social Service Models Helping Address Health Disparities

In one of the largest studies about the impact of social determinants of health initiatives, 544 insurers, hospitals, physician groups and community organizations weighed in on the status of their programs, unmet needs, challenges and recent successes. 51% say they are seeing health-related outcomes from a range of initiatives designed to provide food, housing, education and other social supports but they can be difficult to pinpoint as can demonstrating return on investment to policymakers. It is significant how the behavior of healthcare leaders has changed in the last decade to embrace the role of social services. In this 9-page report, readers can get insight into successful models, study findings, and recommendations for each stakeholder.
